Investment from the Bill & Melinda Gates Foundation has been proposed for a number of diagnostic tools that are pivotal to achieving London Declaration aims.

Why diagnostics are essential to London Declaration control strategies

A decision making tool is needed to prioritise across a range of diagnostics being considered according to cost-effectiveness and public health impact. The priority diseases identified by Foundation staff are trachoma, schistosomiasis, and soil-transmitted helminths (STH). 

Project Methodology

The modelling exercise aims to address the following issues:

  • What is the role diagnostics play in key programmatic goals for each disease area, such as:
    • Guiding the implementation of MDA at the community level;
    • Post-MDA surveillance to prevent disease reintroduction;
  • In the context of these programmatic goals, which diagnostics are likely to have the highest public health impact?
  • What is the required target product profile for these diagnostics?
  • What is the cost-effectiveness of investment in these diagnostics? This evaluation should take into account issues such as:
    • Cost savings if MDA goals are achieved sooner with a diagnostic (e.g. reduction in number of drug donations required)
    • Cost burden of potential reintroduction of disease (e.g. if MDA programs end prematurely due to lack of appropriate diagnostic and need to be re-implemented)

The assessment of health impact should account for the technical feasibility of developing diagnostics with the required target product profiles.

We are convening a series of meetings of modellers with expertise in the first three priority disease areas (trachoma, schistosomiasis and soil-transmitted helminths) to discuss these questions.

Key Research Questions

The primary research question aims to address "which products from the London Declaration should be prioritized for investment to achieve highest public health impact and cost-effectiveness?"
